Prostate Cancer

Introduction
Prostate is defined as “a complex piece of reproductive equipment whose usefulness declines over time, in proportion to its tendency to malfunction”. It is a walnut sized gland located diagonal to the urethra and in front of the rectum. As men become older, this gland may enlarge so as to interfere with urination and causing problems of bladder and kidney. This enlarged prostate is also prone to tumour resulting in what we know as prostate cancer. Prostate cancer is the 2nd most common form of cancers found in men and the 2nd most common cause of their death. Though its progression is so slow that patients live for years and ultimately die of some thing else, but once it has spread to the bones it becomes incurable resulting in extreme pain.

Causes
So far there isn’t any known cause of the prostate cancer. But genetics is said to be one of the major reason why some men experience prostate cancer and the others don’t. These genetically related cancers are usually present at a relatively younger age as well.

Risk Factors
The risk factors for prostate cancer are either unknown or unavoidable. Some that are known are as follows:
  • Age is the major one, prostate cancer happens to one in 53 men of 40s and 50s whereas it happens to one of seven men in there 60s and 70s.
  • It is a genetic disorder, so family history matters a lot.
  • People of Asian-African origin, perhaps because of their darker skin, which produces less vitamin D.
  • Farmers, possibly as a result of their occupational exposure to herbicides.
  • Some evidences even support obesity as putting people on risk.

Diagnosis
In the recent years prostate cancer is diagnosed through a simple test done for prostate specific antigen (PSA), which is protein whose blood level indicates the changes in the prostate. If the reading of the test is above 4, you should be concerned about the disorder. But the reading of the test varies from person to age regarding the age of the patient. For a man in his 40s, even a reading above 2.5 calls for concern. The doctors do not usually consider single reading, a successive measurement is what most of the physicians recommend. If the second test once again confirms high reading, the patient is usually ordered a biopsy. In this case cancer cells are rated on a Gleason scale ranging from 1 to 10, the higher the value the more aggressive and dangerous tumour would be.

Prevention
The only way to prevent Prostate cancer is castration at young age. Regular intake of free-radical scavengers like selenium, vitamin E and A are also said to be beneficial in this regard.

Treatment
The treatment of Prostate cancer is wide and diverse along with many side effects that could be unpredictable as well as unpleasant. Following are some of the treatment options available to the men:
Radical Prostatectomy: This procedure is actually a conventional open surgery that leads to the removal of the prostate. It is said to be the best treatment for the cancer that has not spread beyond the prostate. It has many side effects as well such as partial loss of control over the bladder and sexual impotence. This treatment is the gold standard among all the treatment of the Prostate cancer. The effects of this treatment mostly depend on the skill and experience of the surgeon, so pray that your surgeon is just the right one.
Laparoscopic Surgery: This is a recent method of surgery and is slowly gaining acceptance and fame over the years. The surgery in this case is done through a tine incision, and doctors perform their tasks guided by a tiny camera. This surgery has minimized the loss of blood as well as the scarring. The side effects of this treatment include incontinence, impotence and complications of the bowel.
Radiation: This is done externally through a focused beam or via rice-grain size seed of a radioactive isotope, which is inserted into the prostate. This is the cheapest and the easiest of the three just mentioned. The draw back of this surgery is that it does not ensure complete elimination of all the tumour cells and the damage it causes may give way to future complications in the surgery.
Hormone Therapy: Every man whose cancer has spread beyond the prostate chooses this method of treatment. This treatment is done through drugs that eliminate or at leas block the hormone testosterone that causes the prosperity of the prostate tumour. This treatment is often used with radiation to achieve effective results.
Watchful waiting: Older men are usually recommended to keep a watch on their condition rather opting for any treatment. Since, it is difficult for them to go through the difficult procedures of the surgery and the after effects of the surgery or the treatment.

Dietary Guidelines
  • According to Dr. Dean Ornish limiting fat to 10 percent of the total cholesterol might help in lowering the PSA level. So keep this in mind and consult your physician regarding this.
  • A diet high in phyto-estrogens is also recommended to avoid the risk of Prostate cancer.
